Evidence required :

True copy of income proof/certificate (income limit below 3 lakhs).

True Copy of Ration Card (New Barcoded).

True copy of Aadhaar card of each family member

True copy of Lightbill/ Verabill

If you or your family’s name is not in the list of Pradhan Mantri Janarogya Yojana then you can avail Amritam/Vatsalya Yojana of Gujarat Govt.
